59 facets + 8 facets on girdle = 67
2-fold, mirror-image symmetry

L/W = 1.200 T/W = 0.568 T/L = 0.473
P/W = 0.479 C/W = 0.226
H/W = (P+C)/W+0.02 = 0.726
P/H = 0.660 C/H = 0.312
Vol./W^3 = 0.373

Addition # 1 - A 1.20 L/W preform is a good idea. It makes this rectangle much easier to cut. With a preform you can cut all of the girdle facets first, then cut everything else in to meet. This design will darken a bit.
